About The Position

Serenity Hospice Care is seeking a compassionate, dependable Licensed Practical Nurse (LPN) for a full-time Weekend Baylor role providing overnight hospice continuous care in Monmouth and Ocean Counties, New Jersey. The LPN provides high-quality, patient-centered care to hospice patients requiring continuous care, works under RN supervision, and collaborates with the hospice clinical team and facilities. When continuous care cases are not needed, the nurse must be willing to conduct patient visits and provide wound care.

Requirements

• Current, active New Jersey LPN license.
• Strong clinical, communication, and documentation skills.
• Ability to work all three required overnight shifts: Friday 7:00 p.m.–8:00 a.m. Saturday, Saturday 7:00 p.m.–8:00 a.m. Sunday, and Sunday 7:00 p.m.–8:00 a.m. Monday.
• Reliable transportation.

• Hospice, palliative care, or long-term care experience.

Responsibilities

• Provide nursing care according to each patient's plan of care and under RN supervision.
• Administer medications and treatments as ordered and within the LPN scope of practice.
• Monitor patient comfort, symptoms, and condition, and promptly report changes to the supervising RN.
• Support patients and families with compassion, dignity, and respect.
• Complete accurate, timely clinical documentation.
• Communicate with the hospice clinical team to support continuity of care.
• Conduct patient visits and provide wound care when continuous care cases are not needed.
• Collaborate with facilities to ensure all needs are met.
