About The Position

The LPN provides nursing care to individual patients by using the nursing process to plan, implement, and evaluate care. The role collaborates with other members of the healthcare team, evaluates patient responses to interventions and progress toward goals, and accepts delegated nursing measures within the nurse’s preparation, qualifications, and licensure. Responsibilities include admissions and intake assessments, discharge planning and coordination, patient and family education, care coordination, communication with providers, clinical documentation, and patient support. The position is part-time, 24 hours per week.

Requirements

Graduate from a nationally accredited licensed practical nursing program.
Hold an active Licensed Practical Nurse (LPN) license in Indiana or an active Nurse Licensure Compact (NLC) LPN license.
Hold Basic Life Support certification through the AHA.
Other advanced life support certifications may be required according to unit or department specialty and patient care policies.

Nice To Haves

1–2 years of relevant experience preferred.

Responsibilities

Conduct admissions and intake assessments.

Complete clinical documentation and provide patient support.
Use the nursing process to plan, implement, and evaluate nursing care for individual patients.

Evaluate responses to interventions and patient status toward goal achievement.
Accept delegated nursing measures that the nurse is prepared, qualified, and licensed to perform.
Complete the required orientation.
