About The Position

The Licensed Practical Nurse supports coordinated, team-based care for individuals through partnerships with patients, caregivers and families, community resources, and physicians. The role facilitates a shared-goal model across care settings to provide coordinated, high-quality preventive care that is patient- and family-centered. The LPN supports the nursing process and works under the direction of the Medical Director, Nurse Practitioner, and/or Professional Registered Nurse.

Requirements

• Thorough knowledge of nursing principles and procedures.
• Experience handling multiple telephone lines.
• Excellent clerical and organizational skills.
• Graduate of an accredited School of Practical Nursing.
• Licensed Practical Nurse (LPN) license.
• BLS certification.
• Demonstrate continuing education in accordance with state regulation.
• Working knowledge of Windows and PC applications.
• Ability to work independently and take direction.
• Self-motivated, with the ability to work independently and flexibly within a team framework.
• Excellent oral and written communication skills.
• Ability to handle confidential and sensitive information.

Nice To Haves

• Experience in specimen testing.
• Physician office experience.
• Data entry skills.
