About The Position

Home Care Network, Inc. (HCN) is a nurse-owned and locally led home health agency seeking a highly skilled, experienced, accountable, and self-motivated home health LPN or RN to join our agency as a Home Health Nurse to care for clients in and around Youngstown, OH (Mahoning County) twice-daily on weekends. This role offers autonomy in a positive, client-centered culture with excellent Quality and Compliance support and training. Requirements

  • Active Ohio LPN or RN in good standing.
  • Minimum of 1 year of experience in wound care is required.
  • Must be comfortable working in a home with cats and dogs.
  • Ability to consistently, reliably provide services in Youngstown, OH on weekends is required.
  • Reliable transportation is required.
  • Graduate of an accredited school of professional nursing.
  • Active LPN or RN, with current license to practice as a Licensed Practical Nurse/Registered Nurse in the state of Ohio, is required.
  • Wound care experience is required.
  • At least 1 year in home health industry + 1 year of nursing experience.
  • Minimum of 1 year direct patient experience with vent care is required.
  • Good interpersonal communication skills - written and verbal.
  • Current driver's license or state ID, plus reliable transportation, are required.
  • Excellent attendance record with past employers.
  • Ability to meet all of the basic job requirements is a must.
  • Willingness and ability to pass state and federal background check.

Nice To Haves

  • Paid experience at a home health agency (or equivalent) is preferred.
  • Additional Home Health Nursing experience/ familiarity is highly desired.
  • Active CPR and First Aid certificates highly desired (for speed to start care).
  • 1+ years of experience in the following is desirable but not required: Hoyer lift use and transfers, ROM exercises, trach care, vent care, tube feedings and medication administration, respiratory treatments (incl. nebulizer, shake vest, cough assist), colostomy care, catheter care, Case Management, PICC/PORT/IV Care, lab draws, and OASIS and Non-OASIS assessments.

Responsibilities

  • Provide skilled nursing care under the direction of a Registered Nurse.
  • Communicate appropriate status/condition changes to the RN per agency guidelines.
  • Provide skilled nursing visits for clients as directed by the RN Case Manager.
  • Follow the care plan established by the Registered Nurse.
  • Recognize and report changes in client condition to the Case Manager and physician, as directed.
  • Communicate information on a timely basis, consistent with agency policy and urgency of the situation.
  • Perform documentation in the clinical record.
  • Deliver care to clients as delegated by the RN Case Manager.
  • Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager.
  • Individuals with the license designation of "LPN M-IV" may perform venipuncture for the purpose of obtaining blood samples for laboratory analysis.
  • Promote personal safety and a safe environment for clients and coworkers.
  • Demonstrate knowledge of safety/infection control practices by compliance with policies and procedures.
  • Recognize and respond appropriately to potentially unsafe situations.
  • Demonstrate safe practice in the use of equipment.
  • Make recommendations on improvement of safety.
  • Perform job in compliance with agency policies and procedures and professional and community standards.
  • Accept responsibility for professional nursing role.
  • Follow applicable state and federal laws and agency standards.
  • Attend all meetings as required.
  • Maintain confidentiality in all aspects of the job.
  • Refrain from discussing client or staff identities, conditions, or circumstances in public or with others who do not have a legitimate interest in the person.
  • Participate in the development, implementation, and evaluation of the Agency Quality Improvement Program and pertinent activities.
  • Perform other related duties and responsibilities as deemed necessary.
