About The Position

Sanford Health is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) or Med Aide to join their cottage hospice team. This role involves providing comfort-focused, patient-centered care to individuals and families during a significant life stage. The position requires close collaboration with an interdisciplinary team to support symptom management, emotional comfort, and dignity for patients. The LPN will participate in the implementation and evaluation of patient care for stable and predictable patients of all ages in a home care setting. This includes demonstrating knowledge of nursing principles, performing assessments, interpreting patient status, and providing age-specific care. The role also involves ensuring patient health, comfort, and safety, understanding medication effects, patient education principles, and common safety hazards. Skills in administering treatments and medications are essential.

Requirements

  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
  • Currently holds an unencu m b ered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
  • Obtains and subsequently maintains required department specific competencies and certifications.
  • Basic Life Support (BLS) for health care providers required.

Nice To Haves

  • One year of professional nursing experience preferred.
  • If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.
  • Depending on location, a valid drivers license may be required.

Responsibilities

  • Provide comfort focused, patient centered care to individuals and families during one of the most meaningful stages of life.
  • Work closely with an interdisciplinary team to support symptom management, emotional comfort, and dignity for every patient.
  • Participate in the implementation and evaluation of patient/resident care for stable and predictable patients/residents of all ages in the home care setting.
  • Demonstrate knowledge of nursing principles for the patient/resident in the relevant setting.
  • Perform testing to assess data reflective of the patient's/resident's status and to interpret appropriately the information needed to identify each patient's/resident's requirements relative to the age-specific needs, and to provide the care needed.
  • Demonstrate competency and practice within the full scope of nursing expertise/knowledge and utilize appropriate age and population specific standards as designated in the home care setting.
  • Ensure the health, comfort and safety of patients/residents.
  • Administer a variety of treatments and medications as directed.
