About The Position
Tryfacta is seeking a Licensed Practical Nurse (LPN) to provide quality nursing care in a long-term care setting in Glenwood Springs, Colorado. This is a 13-week travel contract assignment with 12-hour night shifts from 6:00 PM to 6:00 AM and 36 guaranteed hours per week. The role involves monitoring residents’ health, administering medications and treatments, documenting care, and collaborating with the healthcare team to support safe, patient-centered care.
Requirements
- Active LPN license valid for practice in Colorado.
- Familiarity with PointClickCare (PCC) charting.
- Strong communication, organizational, and clinical assessment skills.
- Ability to work 12-hour night shifts reliably.
Nice To Haves
- Long-term care or skilled nursing facility experience preferred.
Responsibilities
- Provide nursing care to residents in a long-term care setting.
- Administer medications and treatments according to provider orders and established protocols.
- Monitor residents’ health status and report changes to the appropriate healthcare team.
- Maintain accurate and timely documentation using PointClickCare (PCC).
- Assist with care planning and coordinate with RNs and other healthcare professionals.
- Follow infection control, patient safety, and confidentiality standards.
