About The Position

Tidewell Hospice, a member of Empath Health, provides compassionate hospice care to families across southwest Florida. The Field LPN serves the Manatee County and North Sarasota County coverage areas, providing bedside clinical observations, prescribed care, and physical and emotional support to patients under the direction of an RN case manager. The LPN collaborates with the RN to accept and process physician orders and uses monitoring, intervention, and teaching skills to promote comfort and quality of life for patients and families.

Requirements

• Graduate of an approved nursing program.
• Hold a current Florida LPN license.
• At least one year of experience in an acute care setting providing direct patient care.

Nice To Haves

• Previous experience as a hospice LPN.

Responsibilities

• Provide continuous bedside clinical observations and prescribed care according to the plan of care and under the direction of the RN case manager.
• Provide physical and emotional support to patients.
• Report significant changes in patient condition and family needs as needed.
• Collaborate with the RN to accept and process physician orders to maximize patient comfort.
• Use monitoring, intervention, and teaching skills to maximize patient comfort and enhance quality of life for the patient and family.
