About The Position

Provides nursing care under the supervision of an RN for assigned patients in assigned areas, using evidence-based nursing as the standard. The position is at Masonicare Health Center in Wallingford, Connecticut, on a day shift for 24 hours per week, including every other weekend. The LPN provides care suited to residents’ or patients’ ages, needs, and abilities, contributes to care planning, communicates with the interdisciplinary team, documents care, supervises CNA-provided care, and participates in education and quality improvement activities.

Requirements

Graduate of an accredited school of nursing for practical nursing.
Current State of Connecticut license to practice as a licensed practical nurse.

Nice To Haves

One or more years of clinical experience in a hospital or similar health care setting is preferred.

Responsibilities

Provide nursing care appropriate to residents’ or patients’ ages, needs, and abilities, including medication and treatment administration, teaching, and implementation of the interdisciplinary care plan.
Contribute to care plan development by collecting and recording data in an ongoing and systematic manner.
Communicate and collaborate effectively with residents or patients, families, colleagues, and interdisciplinary team members to meet resident or patient and unit needs.
Document accurate, pertinent, and comprehensive information in a timely and concise manner in accordance with regulations and facility policies and procedures; competently use the Electronic Medical Record.
Supervise care provided by CNAs to ensure compliance with the interdisciplinary care plan and resident profile.
Participate in educational programs, in-services, meetings, and workshops, and use current literature to enhance professional skills and knowledge; attend all annual mandatory in-services.
Participate in quality improvement programs as directed.
Follow the standards of the Diligent Minimal Lift Program.
Perform other duties as required.
