Requirements

  • Graduate of an accredited Practical Nursing program.
  • Current and unrestricted Licensed Practical Nurse (LPN) license.
  • Current BLS certification required.
  • Minimum two years of nursing experience in primary care, family practice, or internal medicine required.
  • Experience utilizing electronic medical record (EMR) systems required.
  • Strong knowledge of primary care workflows, chronic disease management, preventative care, and patient care processes.
  • Understanding of referrals, prior authorizations, and insurance processes.
  • Excellent communication, customer service, organizational, and time-management skills.
  • Ability to manage multiple priorities while maintaining accuracy and attention to detail.

  • Care coordination experience preferred.
  • Experience with patient intake, referrals, prior authorizations, and clinical documentation preferred.

Responsibilities

  • Conduct comprehensive patient intake assessments and gather medical, medication, family, and social histories.
  • Review, update, and maintain patient information within the EMR.
  • Prepare charts and documentation for provider review prior to appointments.
  • Conduct patient follow-up regarding lab results, diagnostic testing, and provider-directed treatment plans.
  • Coordinate referrals, specialist appointments, medical records, and follow-up care activities.
  • Monitor outstanding labs, imaging studies, referrals, and care plan activities.
  • Assist providers with clinical inbox management, patient messages, and prescription refill requests.
  • Complete prior authorizations and coordinate supporting documentation.
  • Facilitate communication between patients, providers, pharmacies, specialists, and healthcare organizations.
  • Support chronic disease management, preventative care outreach, and population health initiatives.
  • Maintain accurate documentation and ensure compliance with clinical protocols and HIPAA requirements.
  • Support wellness events, health assessments, and other patient outreach activities as needed.
