Under the direction of the Administrator of Providence Home Care, this role facilitates referrals, coordinates staff, and assists patients and families with transitions into home health care. The Intake Specialist communicates with physicians, hospital staff, patients, caregivers, and agency staff to provide continuity of care. Requirements
- Education to meet certification, license or registration requirement
- Oregon Licensed Practical Nurse
- 1 year of LPN experience
