About The Position

Hinds Lifecare, a nonprofit agency in California’s Central Valley and a pioneer in hospice and palliative care, is seeking an LVN Intake Coordinator. This role is responsible for managing all hospice information calls regarding new referrals, facilitating evaluations for hospice service, and overseeing the entire patient intake process. The position also involves conducting Hospice evaluations as directed. Requirements

  • Completion of a Licensed Vocational Nursing Education Program.
  • Demonstrated ability to work with health team personnel.
  • Minimum 1 year experience in acute care facility or community health agency.
  • Current LVN license.
  • Current CPR certification.
  • Recent physical and TB.
  • Tdap, MMR, Varicella, HepB immunizations.
  • Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs).
  • Ability to learn and assimilate the NetSmart Computer Program/System and navigate through multiple technical interface portals to enter and retrieve information related to patient care.
  • Ability to work weekend rotation.

  • One-year hospice experience.
