About The Position

Assists in the coordination of patient care under the direction of the Business Office Manager. Promotes and exemplifies the Company's mission, vision, and values. Receives all intake information and enters it into the computer. Completes intake forms and routes them appropriately for admission approval. Contacts patients to verify correct contact information and that Home Health Care has been ordered by their physician. Communicates with the business development team, discharge planners, and facility contacts to receive new and returning patients in a timely manner. Complies with all areas of the Agency’s Compliance Program and HIPAA regulations. Assists in maintaining case management notes while maintaining the confidentiality of patient records. Provides effective communication to clients, their families, team members, and other healthcare professionals. Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency. Requirements

  • 1 year experience in a general office environment.
  • Ability to communicate verbally and in writing effectively.
  • Demonstrates proven decision-making skills.
  • Must read, write, and comprehend English.
  • Must be able to utilize multiple computer information systems, including electronic medical record systems, electronic patient referral systems, and Microsoft Office.
  • High school diploma or equivalent.
  • Current driver's license in good standing.
  • The employee is responsible for renewing their driver's license before it expires to continue employment.

Nice To Haves

  • Preferred health care experience.
  • Intake Coordinator Level 2- the employee must hold a minimum of an active, unencumbered LVN/LPN licensure.
  • Intake Coordinator Level 2- the employee's responsibility is to maintain their licensure before it expires to continue employment.
