About The Position

Tidewell Hospice, a member of Empath Health, provides compassionate hospice care to families across southwest Florida. The LPN Hospice Field role covers Sarasota and Venice and works Monday through Friday, 8 a.m. to 5 p.m. Under the direction of an RN case manager, the LPN provides clinical observations, prescribed care, and physical and emotional support to patients according to the plan of care. The role also supports families and caregivers, reports significant changes in patient condition and family needs, and collaborates with the RN to process physician orders that maximize patient comfort.

Requirements

• Graduate of an approved nursing program.
• Current Florida LPN license.
• At least one year of experience providing direct patient care in an acute care setting.
• Complete an approved 30-hour IV certification course within six months of hire.

• Previous experience as a hospice LPN.

Responsibilities

• Provide continuous bedside clinical observations and prescribed care under the direction of the RN case manager.
• Provide physical and emotional support to patients according to the plan of care.
• Report significant changes in patient condition and family needs.
• Support families and caregivers, and report psychosocial or spiritual needs to the team.
• Collaborate with the RN to accept and process physician orders to maximize patient comfort.
• Use monitoring, intervention, and teaching skills to maximize comfort and enhance quality of life for the patient and family.
