About The Position
Empath Home Health, a member of Empath Health, is seeking a caring full-time Home Health LPN to provide individualized care in the Ocala, Florida area, including The Villages and Marion, Sumter, and Lake counties. Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, as well as personal care support. The LPN provides nursing services in compliance with agency policies and procedures, state and federal regulations, and the patient's plan of care, under the direction and supervision of a Registered Nurse. The schedule is Monday through Friday, 40 hours per week, with required weekend and holiday rotations.
Requirements
- Active Florida LPN license.
- Graduate of an approved nursing program.
- Requires 1–2 years of LPN experience.
Nice To Haves
- Home health care experience preferred.
- Intravenous (IV) certification strongly preferred.
Responsibilities
- Provide nursing services in compliance with agency policies and procedures, state and federal regulations, and the patient's plan of care under the direction and supervision of a Registered Nurse.
- Observe patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
- Assist the Physician and/or Registered Nurse in carrying out the plan of care and performing specialized procedures.
- Instruct patients and families on diet, medication, and treatment.
- Collaborate with the interdisciplinary team to ensure seamless, holistic care.
