About The Position

Under the supervision of the Director of Clinical Services, the Licensed Practical Nurse (LPN) is responsible for providing day-to-day nursing care to clients. The LPN plans and manages client care according to individual needs and explains care to clients and their families. Duties may include assistance with bathing, dressing, eating, toileting, and daily exercise; changing bandages, dressings, and catheters; and monitoring clients’ conditions, including taking vital signs. Other duties may be assigned.

Requirements

• Graduate of an accredited LPN school.
• Current LPN licensure in Oklahoma.
• Current Basic Cardiac Life Support (BCLS) certification.
• One to two years of recent Home Health LPN experience.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Medical knowledge, patience, compassion, kindness, critical-thinking skills, knowledge of infection-control practices, stamina, physical endurance, observational skills, knowledge of occupational hazards and safety rules, communication skills, leadership, and empathy.

Nice To Haves

• Current Advanced Cardiac Life Support (ACLS) certification.

Responsibilities

• Record client histories.
• Take and record vital signs, including blood pressure, temperature, and weight.
• Enter information into computer systems and update client records confidentially.
• Ensure clients and families understand release instructions.
• Monitor clients’ fluid and food intake and output.
• Move clients safely.
• Observe client reactions to medications and monitor mental health status.
• Provide emotional support to clients and families.
• Observe clients for skin breakdown.
• Maintain clients’ cleanliness and comfort, including bathing, feeding, and dressing.
• Evaluate client wellbeing and report needs to medical staff.
• Explain treatments and procedures to clients and families.
• Change dressings and bandages and ensure clients are healing properly.
• Perform other duties as assigned.
