LPN Field Nurse | Home Health Patient Care
About The Position
This role involves providing skilled nursing visits to patients in their homes within a 35-mile radius of the nurse's home. The LPN Visit Nurse will assess how patients are managing their health at home, determine reasons for non-adherence to care plans, provide wound care, educate patients and caregivers, reinforce education, and use nursing instincts to identify issues beyond the initial reason for the visit. The workday occurs in the community, with travel between patient homes, offering independence while maintaining connection to RN Case Managers and other clinical staff.
Requirements
- Active Minnesota Licensed Practical Nurse (LPN) license
- Strong clinical observation and patient-care skills
- Ability to work independently and make sound clinical decisions
- Excellent patient and caregiver communication
- Strong organization and time management
- Compassionate, dependable approach to nursing
- Reliable vehicle, valid driver's license, and current auto insurance
- Ability to travel to patients within approximately a 35-mile radius of your home

Responsibilities
- Provide wound care and dressing changes
- Manage and educate patients on medications
- Perform skilled nursing observations
- Administer injections and ordered treatments
- Monitor chronic diseases
- Educate patients and caregivers
- Reinforce physician orders and discharge instructions
- Monitor for changes in condition
- Identify potential safety or care concerns in the home
- Communicate clinical changes to RN Case Managers and physicians
- Coordinate with therapists and other care team members
- Complete accurate and timely clinical documentation
- Ensure patient care continues working after the visit
