About The Position

Under the direction of the Registered Nurse and/or Director of Clinical Services, evaluates needs and implements care according to the plan of care established for the patient. The LPN will participate in the provision of Continuous Care, facilitating medical and emotional support to patient and family during a crisis or unstable situation.

Requirements

  • Graduate of an approved school of nursing.
  • Current licensure in the State of Kentucky.
  • Two years experience with medical-surgical patients in hospital, nursing home, or home health setting.
  • Must be able to access patient’s home independently.

Responsibilities

  • Provides direct nursing care to patients in order to maintain highest level of comfort and quality of life.
  • Functions within the standards of the Nurse Practice Act.
  • Assists RN Case Managers in the management of their caseloads and in responding to patient/family needs in accordance with the plan of care.
  • Visits patients as assigned and evaluates physical, emotional and clinical status.
  • Provides nursing, hygienic, and personal care as directed by the RN Case Manager and outlined by the plan of care.
  • Communicates clinical and personal findings to appropriate team members, including: RN Case Manager, Nursing Coordinator, Social Worker, and Chaplain.
  • Documents observations and nursing care provided in the patient’s clinical record, assuring accuracy, completeness, and timeliness.
  • Assists and guides the home health aide in direct patient care.
  • Complies with physician orders; clarifies with RN orders that are unusual or inappropriate.
  • Reinforces patient and family teaching regarding basic nursing procedures as indicated.
  • Provides a safe patient care environment, i.e., drug disposal, sharps disposal.
  • Participates as a member of the IDT in meeting the needs of the total patient population.
  • Attends weekly IDT meetings, facilitating discussion of issues from caseload for full staff discussion, consultation and evaluation.
  • Participates with on-call schedule to provide twenty-four hour, seven day a week coverage.
  • May provide bereavement support to families by attending visitation or funeral services when possible.
  • Participates in the provision of Continuous Care as assigned to provide medical and emotional support to patient and family during a crisis or unstable situation.
  • May be required to work in the inpatient unit.
  • Contributes to the overall success of Hospice by cooperating in other organizational requirements and activities.
  • Attends and participates in staff and team meetings.
  • Communicates information concerning work schedule and need for time off to Director of Clinical Services.
  • Demonstrates commitment to further professional education by attendance at HLC in-service programs and other offerings as available.
  • Participates in organizational committees and special projects as assigned.
  • Demonstrates familiarity with Hospice policies and State and Federal regulations governing provision of Hospice care.
