About The Position

Trustbridge Hospice, part of Empath Health, is seeking a caring and motivated LPN Coordinator to join our team in West Palm Beach. This role is ideal for an LPN who is looking for a consistent weekend-only schedule that allows for flexibility during the week while making a meaningful impact. The schedule will be: every Friday/Saturday/Sunday 6:00am – 6:30pm. The ideal candidate will play a vital role in ensuring seamless coordination of care by supporting staff visits and collaborating with interdisciplinary team members as well as dispatching emergent visits. You’ll act as a central connector—helping teams respond efficiently and compassionately to patient and family needs. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

Requirements

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Minimum experience of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing.
• Understanding of the ability to apply the characteristics of hospice nursing, case management, crisis intervention, communication skills, thanatology skills and needs.

Nice To Haves

• Basic computer skills and EMR software experience a plus.

Responsibilities

• Coordinate with Managers and care team members to support patient and family direct and indirect care needs
• Provide quality professional intervention guided by established policies and protocols
• Collaborate closely with management, physicians, and field staff using a teamwork-driven approach
• Prioritize patient comfort and family-centered care in every interaction
• Deliver consistently personalized, sincere, and engaging service to patients and families
• Support and promote complete patient and family satisfaction
