As an LPN Central Admissions, you will facilitate the timely and appropriate initiation of Home Health, Hospice and Infusion Pharmacy services for all referred patients. A caregiver in this position works days from 9:00 AM – 5:30 PM. This is a hybrid position requiring training on site in Independence, OH for about 4-6 weeks and frequent work on site.

Requirements

Graduate from an accredited Practical Nursing Program
Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)

One year of clinical experience in Homecare

Responsibilities

Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered Nurse.
Coordinate initial and final orders on internal and external referrals for patients needing home care nursing, therapy and social work services.
Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches when appropriate.
Communicate the home care needs with scheduling, managers, reimbursement and other parties involved in the home care services process.
Serve as the CCHCS information specialist.
Understand other services and products available through CCHCS, including criteria for service and reimbursement for all the home care business lines.
