LPN Care Coordinator | 39 PTO Days | No Weekends
About The Position
Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Licensed Practical Nurse (LPN) Coordinator in Beverly, Massachusetts. This is a PACE-focused LPN coordination role supporting older adults within an outpatient and home-based care environment. It offers the opportunity to build long-term relationships while coordinating care and supporting patient independence. The role is primarily clinic-based with occasional home visits as needed, within a collaborative interdisciplinary environment working closely with Occupational Therapy and nursing teams. It is an autonomous role assessing patient needs and recommending care plan updates.
Requirements
- Active Massachusetts LPN license REQUIRED
- CPR certification REQUIRED
- Comfortable working with geriatric patient population REQUIRED
- Ability to assess medical and functional patient changes REQUIRED
- Strong communication and interdisciplinary collaboration skills REQUIRED
- Comfortable with outpatient coordination and occasional home visits REQUIRED
Responsibilities
- Coordinate home care and outpatient support services for geriatric patient population
- Assess patient medical and functional changes
- Recommend updates to patient care plans
- Collaborate with Occupational Therapy and interdisciplinary care teams
- Support continuity of care and patient independence goals
- Conduct home visits as needed within care coordination structure
