About The Position

The LPN- Cardiopulmonary Rehab plays a vital role in a multidisciplinary team dedicated to the recovery and long-term wellness of patients with chronic cardiac and pulmonary diseases. Under the supervision of a Registered Nurse (RN) or Physician, the LPN provides direct clinical care, monitors patient physiological responses during exercise, and delivers essential education to help patients manage their conditions. This role involves patient intake, medication review, vital sign documentation, assisting with new patient evaluations and care plans, monitoring patients during rehab for safety, providing cardiac and pulmonary patient education, reinforcing lifestyle and medication adherence, and documenting care in Epic. The LPN will also communicate effectively with the interdisciplinary team, assist with care coordination, scheduling, and follow-up, and actively support the mission, vision, and values of Southeast Health and its department. The position is part-time (0.5 FTE) and is a Per Diem role within Southeast Health, an academic community-based health system serving 460,000 residents in southeast Alabama, southwest Georgia, and the Florida panhandle. Southeast Health includes a 420-bed hospital, the region’s largest medical group, and the Alabama College of Osteopathic Medicine (ACOM). The organization is not-for-profit and aims to promote healing, prevent disease, educate future medical professionals, and enhance wellness and prosperity in the region.

Requirements

  • Graduate of an accredited school of nursing
  • Current, unrestrictive LPN license in the state
  • 1 year med surg experience practicing as LPN
  • Maintain current and active LPN licensure in the state of Alabama
  • Successful course completion in Advanced Cardiac Life Support within 6 months of hire
  • Maintain current and active ACLS certification
  • Successful course completion in Basic Cardiac Life Support
  • Maintain current and active BLS certification
  • Successful course completion of institutional basic EKG course (interpretation of life-threatening dysrhythmias)
  • Must be able to read, write, and speak English
  • Well-versed in medical terminology and simplify medical jargon to help patients understand their condition and treatments
  • Experience in operating personal computers, including operation of computer software in a Windows environment
  • Demonstrates responsibility for educational requirements as evidenced by reading all assigned related references, and attending all required educational meetings, or webinars, and completing annual Symplr requirements

Responsibilities

  • Perform patient intake, including history and medication review.
  • Obtain and document vital signs, report abnormalities.
  • Assist with new patient evaluations and care plans.

  • Reinforce lifestyle and medication adherence guidance.
  • Document care in Epic accurately and timely.
  • Communicate effectively with the interdisciplinary team.
  • Assist with care coordination, scheduling, and follow up.
  • Actively support the mission, vision, and values of the hospital and department.
  • Must be able to perform CPR.
  • Performs other duties as requested by the team leader that do not compromise moral code of conduct or protocols set in place for patient or employee safety.
