The LPN- Cardiopulmonary Rehab plays a vital role in a multidisciplinary team dedicated to the recovery and long-term wellness of patients with chronic cardiac and pulmonary diseases. Under the supervision of a Registered Nurse (RN) or Physician, the LPN provides direct clinical care, monitors patient physiological responses during exercise, and delivers essential education to help patients manage their conditions. This role involves patient intake, medication review, vital sign documentation, assisting with new patient evaluations and care plans, monitoring patients during rehab for safety, providing cardiac and pulmonary patient education, reinforcing lifestyle and medication adherence, and documenting care in Epic. The LPN will also communicate effectively with the interdisciplinary team, assist with care coordination, scheduling, and follow-up, and actively support the mission, vision, and values of Southeast Health and its department. The position is part-time (0.5 FTE) and is a Per Diem role within Southeast Health, an academic community-based health system serving 460,000 residents in southeast Alabama, southwest Georgia, and the Florida panhandle. Southeast Health includes a 420-bed hospital, the region’s largest medical group, and the Alabama College of Osteopathic Medicine (ACOM).
