About The Position

Kline Galland is hiring two full-time Long Term Care Social Workers to join its Long Term Care Unit. The role combines social services coordination with therapeutic and meaningful resident activities, helping enrich the lives of older adults and create a supportive, engaging environment. Social Workers collaborate with residents, families, staff, and interdisciplinary teams to support resident goals and quality of life. The schedule is 40 hours per week, with positions available Monday through Friday or Tuesday through Saturday.

Requirements

• Degree in Social Work or a related field of study.
• Two years of experience in Human Services or an applicable field.
• Ability to manage multiple priorities and meet deadlines for time-sensitive responsibilities.
• Ability to manage changing priorities each day.
• Strong verbal and written communication skills.
• Strong collaboration and follow-through skills.
• Computer skills for charting and documentation.
• Ability to work effectively as part of a team.
• Passion for serving older adults and engaging with residents, families, and team members.

Nice To Haves

• Prior experience in activities is a plus.

Responsibilities

• Conduct comprehensive psychosocial assessments.
• Develop and maintain individualized care plans in collaboration with the interdisciplinary team.
• Coordinate engaging activity programs and maintain required unit documentation.
• Provide resource referrals and collaborate with residents' families to support resident and family goals.
• Provide crisis intervention and therapeutic support as needed.
• Conduct resident interviews to assess mood, behavior, and cognitive functioning, and complete summaries of findings.
• Educate unit staff and provide person-centered intervention strategies for residents' mood, behavioral, and cognitive concerns.
• Partner with nursing leadership and help guide the interdisciplinary unit team.
• Build relationships with residents, families, and team members.
