About The Position
Kline Galland Home is seeking a compassionate and collaborative Social Worker (Discharge Planner) to join its experienced team on the Transitional Care Unit (TCU). The TCU provides short-term, post-hospitalization rehabilitation to help patients transition from the hospital to their homes or another level of care. As part of the interdisciplinary team, the Social Worker works with physicians, nurses, therapists, patients, and families to support safe, effective, patient-centered discharge planning. The primary role is to develop a safe discharge plan in coordination with the patient, family, and interdisciplinary team. The position includes a $5,000 sign-on bonus.
Requirements
- Degree in Social Work or a related field of study.
- Two years of experience in Human Services or an applicable field.
- Ability to manage changing priorities each day.
- Strong verbal and written communication skills.
- Computer skills for charting and documentation.
- Strong team-player skills.

Responsibilities
- Create safe discharge plans in coordination with patients, families, and the interdisciplinary team.
- Complete required assessments, insurance reviews, and medical chart documentation.
- Communicate frequently with the interdisciplinary team and coordinate meetings.
- Manage a caseload from admission through discharge.
- Maintain effective, timely communication with patients and their families.
- Understand and meet government documentation requirements.
