About The Position

Kline Galland Home is seeking a compassionate and collaborative Social Worker (Discharge Planner) to join its Transitional Care Unit (TCU), which provides short-term, post-hospitalization rehabilitation and helps patients transition from the hospital to home or another level of care. The Social Worker works with physicians, nurses, therapists, patients, and families to support safe, effective, patient-centered discharge planning. The primary role is to develop a safe discharge plan in coordination with the patient, family, and interdisciplinary team.

Requirements

Degree in social work or a related field of study.
Two years of experience in human services or an applicable field.
Ability to manage changing priorities each day.
Strong verbal and written communication skills.
Computer skills for charting and documentation.
Strong team player.

Responsibilities

Complete required assessments, insurance reviews, and medical chart documentation.
Communicate with the interdisciplinary team and coordinate meetings.
Manage the caseload from admission through discharge.
Maintain effective, timely communication with patients and their families.
