Long Term Care Physician – Post-Acute Care Doctor
About The Position
MedCap Health is expanding into the community and is hiring a full-time Long Term Care Physician to help build a high-performing clinical team and deliver post-acute and long-term care. The physician will lead care teams in skilled nursing facilities, manage complex cases discharged early from hospitals, and collaborate with Nurse Practitioners and specialists to provide hospital-level care in a long-term setting. The role focuses on continuity, quality, and compassionate patient interactions. Requirements
- Board certification or board eligibility in internal medicine, geriatrics, critical care, or family practice.
Nice To Haves
- Previous experience as a hospitalist or in leadership positions is strongly preferred.
