Long Term and Post-Acute Care APP
About The Position
MedCap Health is seeking a full-time Long Term and Post-Acute Care APP to provide care in post-acute settings. The role offers flexibility, autonomy, and administrative support, along with a competitive salary and malpractice coverage. The schedule is Monday through Friday on day shifts, with no overnight or weekend work. As a member of a multidisciplinary team, the APP manages patients with acute and chronic diseases and collaborates directly with the attending physician to provide high-quality care. The role includes managing patients with debilitating chronic diseases to help improve their quality of life and coordinating with the Director of Nursing, social workers, and other team members to support comprehensive care.
Requirements
- Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP).
- At least 1 year of experience in post-acute, skilled nursing facility (SNF), or long-term care (LTC) settings.
- Prior experience in post-acute care, long-term care, and/or a skilled nursing facility.
Responsibilities
- Manage patients with acute and chronic diseases.
- Collaborate directly with the attending physician to provide high-quality care.
- Manage patients with debilitating chronic diseases and help improve their quality of life.
- Communicate and coordinate with the Director of Nursing, social workers, and other team members to provide cohesive, comprehensive care.
