Long Term and Post-Acute Care APP
About The Position
MedCap Health is seeking a full-time Long Term and Post-Acute Care APP to provide care in post-acute settings. The APP manages patients with acute and chronic diseases and collaborates directly with attending physicians to deliver high-quality care, including for patients with debilitating chronic diseases. The role is part of a multidisciplinary team and involves working with the Director of Nursing, social workers, and other team members to coordinate comprehensive care. The schedule is Monday through Friday on day shifts, with no overnight work or weekends. MedCap Health describes the role as offering flexibility, autonomy, administrative support, and a sense of purpose.
Requirements
- Board-certified geriatric, family, or adult Nurse Practitioner; certifications listed include ANCC, ACHPN, or AANP.
- At least 1 year of experience in post-acute, skilled nursing facility, or long-term care.
- Prior post-acute, long-term care, and/or skilled nursing facility experience.
Responsibilities
- Manage patients with acute and chronic diseases.

- Help manage patients with debilitating chronic diseases and improve their quality of life.
- Communicate and coordinate with the Director of Nursing, social workers, and other team members to support comprehensive patient care.
