Long Term and Post-Acute Care APP
About The Position
MedCap Health is hiring a Long Term and Post-Acute Care APP to provide care in post-acute settings. The position may be full-time or part-time and offers schedule flexibility, autonomy, and administrative support. The APP manages patients with acute and chronic diseases and collaborates with the attending physician to provide high-quality care, including care for patients with debilitating chronic diseases. The role also involves working with the Director of Nursing, social workers, and other members of a multidisciplinary team to coordinate comprehensive patient care. MedCap Health is a physician-led organization providing medical services to more than 200 skilled nursing facilities across 11 states. Its services include pulmonology, nephrology, psychiatry, and internal medicine. Requirements
- Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP).
- At least 1 year of experience in post-acute, skilled nursing facility, or long-term care.
- Prior experience in post-acute care, long-term care, and/or a skilled nursing facility.
Responsibilities
- Manage patients with acute and chronic diseases.
- Collaborate directly with the attending physician to provide high-quality care.
- Help manage patients with debilitating chronic diseases and improve their quality of life.
- Communicate and collaborate with the Director of Nursing, social workers, and other team members to provide coordinated, comprehensive care.
