Long Term and Post-Acute Care APP
About The Position
MedCap Health is seeking a full-time Long Term and Post-Acute Care APP to provide care in post-acute settings. As part of a multidisciplinary team, the APP manages patients with acute and chronic diseases and collaborates directly with the attending physician to deliver quality care. The role includes managing patients with debilitating chronic diseases and supporting improvements in their quality of life. The APP works closely with the Director of Nursing, social workers, and other team members to coordinate a cohesive approach to care. The schedule is Monday through Friday, day shifts, with no overnight or weekend work required. MedCap Health describes itself as a physician-led organization that provides a range of medical services and partners with more than 200 skilled nursing facilities across 11 states.
Requirements
- Board certification as a geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP).
- At least 1 year of experience in post-acute, skilled nursing facility, or long-term care settings.
- Prior experience in post-acute care, long-term care, and/or a skilled nursing facility.
