About The Position

The LIGHT-House Program is part of Hesed House’s supportive housing ministry and serves individuals and families who are transitioning from homelessness toward stability and independence. The LIGHT-House Case Manager provides ongoing supportive case management, helping participants set and achieve goals related to housing, employment, health, and overall self-sufficiency. This role involves coordinating services, offering guidance, and connecting participants to community resources that promote long-term success. As part of Hesed House’s mission to end homelessness—one person, one family at a time—the LIGHT-House Program provides a safe, stable living environment and individualized support that empowers residents to rebuild their lives and sustain permanent housing.

Requirements

  • Bachelor's degree in human service or a related field (Required).
  • Be able to assess and intervene appropriately in crisis situations.
  • Be compassionate, yet firm.
  • Must ascribe to the Hesed mission: "To feed the hungry, clothe the naked, shelter the homeless and give people the chance to hope again."
  • Must have good verbal and written communication skills.
  • Be self-motivated.
  • Work in a fast paced, sometimes chaotic environment.
  • Have the ability to work flexible hours, including nights, holidays, and weekends.
  • Must pass DCFS and criminal background checks
  • Must possess a valid driver's license.

Nice To Haves

  • An understanding of poverty and 'street smarts' preferred.
  • Experience working with the homeless or vulnerable populations preferred.
  • Bilingual in English/Spanish is a plus.

Responsibilities

  • Responsible for maintaining the safety and security of the participants and the unit.
  • Must exhibit professional conduct within the work environment by demonstrating tact, diplomacy and a professional image, maintain confidentiality and not fraternize with residents and/or guests.
  • Establishes goals with the program participants and potential participants, provides case management, counseling, records case notes, and effectively tracks progress using SP5.
  • Provides tools and services necessary for the participants to achieve goals and advocates for the participant when needed.
  • Actively engages participant in community activities and teaches participant life skills essential to their well-being.
  • Responsible for connecting the participant to community resources and outside services.
  • Carries out duties and responsibilities in a timely and indiscriminate manner.
  • Responsible for making sound decisions in stressful situations with minimal supervision.
  • Assists in assigned work responsibilities and other duties fundamental to the upkeep, maintenance, and well-being of both the CRC and the organization.
  • Ensures that participants turn in all utility bills on time where applicable.
  • Assists in the identification of chronically homeless individuals through both assessments and intakes, completed in the CRC, at PADS OES/AM, and/or in the field, which will most likely require some evening and weekend hours.
  • Responsible for cleaning and preparing units for occupancy, including unit cleanouts, and assisting with team-based cleaning projects such as rest remediation, insect treatments, and bed bug cleanings.
  • Responsible for moving, assembling, and setting up participants with furniture and furnishings in units as needed.
  • Provides transportation for participants to and from important meetings.
  • Provides assistance to other Hesed programs and ministries as needed/requested.
  • Attends various program meetings.
  • Effectively utilizes Microsoft programs including Word, Excel, Outlook, and PowerPoint.
