About The Position

Community Partners in Action (CPA) is seeking a Case Manager for the Waterbury Transitional House (WTH) program. WTH provides men with a supervised, structured, residential environment that is safe and supportive. The program aims to enhance residents' efforts to effect positive changes in behavior and decision-making, promoting responsibility and accountability while reinforcing values of tolerance, respect, cooperation, and concern. CPA is committed to criminal justice reform and has a strong Belonging, Equity, Diversity, and Inclusion (BEDI) culture.

Requirements

  • Bachelor’s degree plus one year experience in conducting assessments and case management duties. Equivalent work experience will be considered in lieu of education.
  • Knowledge of criminal justice and court systems.
  • Ability to interact with people of diverse backgrounds.
  • Strong written and oral communication skills.
  • Strong computer skills.
  • Valid driver’s license and reliable transportation.

Nice To Haves

  • Experience in conducting assessments and case management functions.
  • Bi-lingual (English/Spanish).

Responsibilities

  • Conduct resident intake, admission, and assessment (LSI-R) using motivational enhancement strategies.
  • Develop the Residential Individual Service Plan based on assessment outputs.
  • Provide recommendations for the coordination and sequencing of program services for residents.
  • Recognize and address resident ambivalence and resistance and help residents maintain motivation to change.
  • Collaborate with AIC, Adult Behavioral Health services, Advanced Supervision and Intervention Support, and other community-based services.
  • Coordinate and follow up on resident referrals to community-based services, as necessary and appropriate.
  • Monitor resident behaviors and implement program behavior management system on a consistent basis.
  • Prepare accurate and timely progress and discharge reports.
  • Collaborate with referral sources.
  • Participate in weekly Service Team Meetings.
  • Supervise urine tests as required.
  • Administer initial PREA assessment within 24 hours of admission and PREA reassessment within 30 days.
  • Complete necessary case documentation on each resident in a timely manner, as established by the Program Manager.
  • Input resident data into the Contractor Data Collection System (CDCS) and maintain, update, and utilize the data as directed by the Program Manager.
  • Utilize CPA’s case management system (Securmanage) as directed by the Program Manager.
  • Serve as a role model to clients and staff and model pro-social behaviors.
  • Promote a culture of respect, empathy, mutual support, belonging and work to eliminate any unconscious bias or discrimination in our beliefs, rules and systems.
  • Remain current on research and literature reviews with “what works” in changing offender behavior and recidivism reduction.
  • Attend and participate in all training and booster sessions related to position, as directed by Program Manager.
  • Solicit resident satisfaction feedback.
  • Establish appropriate boundaries with residents and significant others.
  • Participate in quality assurance review of resident sessions and assessment.
  • Participate in related seminars, panels, and workshops to enhance professional development visibility of agency.
  • Perform all other duties as required.
