The Case Managers are responsible for performing a variety of tasks and activities that are designed to effect the lasting rehabilitation of residents including; (1) initial and ongoing assessment of residents; (2) monitoring and evaluation resident level of functioning; (3) development and review of individualized service plans with residents; and (4) delivery of supportive services including harm reduction in accordance with the resident’s need, desires and goals and for acting as an advocate fo the resident. Case managers are also responsible for working as a member of the service team and for having a working knowledge about the epidemiology of AIDS, clinical drug trials, CPR, First Aid, infection contral and OSHA guidelines. Case Managers maintain an active case load of single adults. This is an exempt position.
