About The Position

Elara Caring is hiring a Licensed Vocational Nurse to provide home health care. The LVN follows each patient’s individualized care plan and physician orders, monitors progress, educates patients and caregivers, and coordinates care with the RN Case Manager and broader care team. The role supports positive patient outcomes and helps prevent avoidable hospitalizations and emergency room visits. Travel between patient homes and assigned work locations is required, including in Southwest Houston, Medical Center, Missouri City, Richmond, Rosenburg, and Sugar Land.

Requirements

• Current state license as a Licensed Vocational Nurse or Licensed Practical Nurse.
• Graduate of an accredited certificate, diploma, or associate school of nursing.
• One year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Experience in a hospice or home health environment is preferred.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
