About The Position

Elara Caring is hiring a full-time Home Health Licensed Vocational Nurse (LVN) in Harker Heights, Texas. The LVN provides skilled, individualized nursing care in patients’ homes, working with an RN Case Manager and an interdisciplinary clinical team to help patients manage their health, recover, and remain safely at home. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with travel throughout the assigned territory. The role includes one-on-one patient visits, caregiver guidance, care coordination, and timely documentation.

Requirements

• Active Texas LVN license.
• Graduation from an accredited nursing program.
• At least one year of clinical nursing experience.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Ability to travel throughout the assigned territory.
• Ability to sit, stand, bend, lift, and move intermittently, and lift 50–100 lbs.

Responsibilities

• Provide skilled nursing care in patients’ homes according to individualized care plans and physician orders.
• Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
• Educate patients and caregivers on medications, self-care, and ways to support recovery and independence.
• Partner with the RN Case Manager and clinical team as patient needs evolve.
• Complete accurate, timely documentation of visits, nursing interventions, and medication information.
• Communicate with physicians, families, and caregivers to support coordinated care.
• Help prevent avoidable hospitalizations through attentive care and early recognition of concerns.
