About The Position

Adaptive Aids, LLC is seeking a Licensed Vocational Nurse (LVN) to provide direct nursing care and support within our Home and Community-based Services (HCS) program. This role is ideal for a dependable, detail-oriented nurse who is passionate about delivering quality care in community settings. Under the supervision of a Registered Nurse, the LVN provides direct patient care, medication administration, documentation, and coordination of services to support individualized care plans and ensure compliance with HCS and state regulations.

Requirements

  • Active Texas LVN license in good standing
  • Current CPR/BLS certification
  • Valid Texas Driver’s License or State ID
  • Ability to pass background and Nurse Aide Misconduct Registry checks
  • Reliable transportation
  • Strong communication and organizational skills
  • Ability to lift, transfer, and reposition clients as needed
  • Ability to stand, walk, bend, and perform physical care giving tasks for extended periods
  • Ability to respond to emergency situations quickly and appropriately

Nice To Haves

  • Minimum 1 year of nursing experience
  • HCS, IDD, or home health experience

Responsibilities

  • Provide nursing care in a manner that promotes client safety and comfort
  • Administer medications and treatments (e.g., wound care, catheter care, vitals monitoring)
  • Monitor and report changes in client condition to the RN and care team
  • Assist in developing and implementing individualized care plans
  • Coordinate services such as therapy appointments and follow-ups
  • Communicate effectively with physicians, families, and interdisciplinary teams
  • Support direct care staff in emergency situations and proper care procedures
  • Complete accurate and timely documentation in electronic systems
  • Ensure documentation meets regulatory, legal, and audit standards
  • Maintain accuracy, timeliness, and completeness in all records
  • Follow all infection control protocols and universal precautions
  • Ensure compliance with HCS regulations, HHSC requirements, and company policies
  • Perform duties within LVN scope of practice under RN supervision
  • Follow all delegation instructions and assigned care plans
  • Maintain reliable attendance and punctuality
  • Work across multiple clients or locations as assigned
  • Maintain professional boundaries and ethical conduct at all times
  • Support compliance with HCS program standards and regulations
