About The Position
Family Health Centers of San Diego (FHCSD) is a large, federally qualified health center (FQHC) with a mission to provide comprehensive, affordable, high-quality healthcare and supportive services to all individuals, with a particular focus on the uninsured, low-income, and medically underserved populations. Operating over 90 sites across San Diego County, FHCSD offers a wide array of services including primary care, behavioral health, dental, vision, substance use treatment, and supportive services for the unsheltered. As one of the largest behavioral health services providers in San Diego County, the Mental Health Department is expanding and seeking a Licensed Therapist to join their team. The role emphasizes providing accessible mental health services and addressing the whole person, encompassing both physical and mental well-being.
Requirements
- Must have an active registration in good standing with the State of California as a Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselors (LPCC) or Clinical Psychologist.
Responsibilities
- Provide counseling sessions to assigned patients
- Complete all required documentation accurately and timely
- Performs initial assessment and on-going care
- Participates in the development of individualized care plans
- Addresses co-occurring disorders
- Work as a member of the multi-disciplinary treatment team
- Coordinate with Case Managers and patient support staff
- Connect patients to internal/external resources as appropriate
- Performs other duties as assigned
