About The Position

PrimeCare Medical is seeking a Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW or LCSW) to work as a Behavioral Health Professional in the medical department at the Indiana County Jail in Indiana, Pennsylvania. This is a full-time position with a Monday–Friday schedule from 8:00 a.m. to 4:30 p.m.

The Behavioral Health Professional conducts clinical and risk assessments, develops treatment plans, coordinates referrals and placements, monitors mental health services and standards of care, participates in quality assurance, provides staff education, and prepares monthly performance reports.

Requirements

• Must be licensed as an LPC, LSW, or LCSW in the Commonwealth of Pennsylvania.
• Master’s degree or greater in Social Work, Psychology, or a related Human Service field, with a concentration in clinical coursework, training, and/or experience.
• Minimum of two years of experience.

Responsibilities

• Conduct clinical assessments based on referrals and self-referrals, documenting findings that include at minimum a mental status examination, case conceptualization, diagnosis, and treatment plan.
• Refer patients to psychiatry when appropriate.
• Conduct risk assessments using structured interviews and risk assessment tools.
• Manage inmates at risk for suicide or self-harm according to existing protocols and current policies and procedures.
• Serve as PrimeCare Medical’s mental health liaison for mental health services outside the facility.
• Facilitate admission and placement of inmates or patients in forensic hospitals when necessary.
• Monitor all mental health services provided.
• Monitor clinical services to ensure standards of care comply with NCCHC and accepted psychiatric and psychological standards.
• Serve as a member of the Quality Assurance Committee.
• Develop and conduct in-service education programs according to the annual in-service calendar.
• Collect data and prepare monthly performance reports.
