About The Position

Community Care, Inc. is hiring a compassionate and dedicated Licensed Practical Nurse (LPN) to provide one-on-one private duty nursing care to a 32-year-old adult female in a supportive home setting in Uniontown, Pennsylvania. The role focuses on personalized, family-centered care for a client requiring tracheostomy and ventilator care. The schedule is Monday through Friday, either 3:00 PM–11:00 PM or 11:00 PM–7:00 AM; weekend shifts are also available.

Requirements

• Current LPN or RN license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least six months of nursing experience.
• CPR certification.
• Reliable transportation and a valid driver's license.
• A caring approach, strong independence, and a passion for client care.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Assist with activities of daily living (ADLs).
• Provide tracheostomy and ventilator care and monitoring.
• Administer medications safely and accurately.
• Communicate clearly and compassionately with the care team and family.
• Deliver exceptional, family-centered client care.
