About The Position

Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse to provide one-on-one private duty nursing care to a 33-year-old male adult in a supportive home setting in Charleroi, Pennsylvania. The role focuses on personalized, family-centered care and building a meaningful connection with the client.

This is a full-time evening position, Monday through Friday, from 3:00 PM to 11:00 PM. Community Care is a family-owned home health agency that has served local communities since 1987.

Requirements

• Current LPN or RN license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience.
• CPR certification.
• Reliable transportation and a valid driver's license.
• A caring attitude, strong independence, and a passion for client care.

Nice To Haves

• Home health experience is preferred, but not required.

Responsibilities

• Assist with activities of daily living (ADLs).
• Administer medications safely and accurately.
• Communicate clearly and compassionately with the care team and family.
• Deliver exceptional, family-centered client care.
