About The Position

Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse (or Registered Nurse) to provide one-on-one pediatric private duty nursing care to a 19-year-old male in a supportive home setting in Johnstown, Pennsylvania. The role focuses on personalized, family-centered care and building a meaningful relationship with the individual and family.

This is a full-time evening position, Monday through Friday, from 4:00 PM to 11:00 PM. Community Care is a family-owned home health agency that has served local communities since 1987.

Requirements

• Current LPN or RN license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience; home health experience is preferred but not required.
• CPR certification.
• Reliable transportation and a valid driver's license.
• A caring heart, strong independence, and a passion for pediatric care.

Responsibilities

• Assist with activities of daily living (ADLs).
• Perform g-tube care and feedings, and follow aspiration precautions.
• Monitor seizures.
• Administer medications safely and accurately.
• Communicate clearly and compassionately with the care team and family.
• Deliver exceptional, family-centered pediatric care.
