About The Position

Rocky Mountain Care - Clearfield is seeking a Licensed Practical Nurse to provide long-term care in its Clearfield, Utah facility. The position delivers nursing care to a group of residents in a nursing unit, including assessing resident needs, developing individual care plans, administering nursing care, and supervising Certified Nursing Assistants and other personnel involved in care delivery. Requirements

• Hold a current, unencumbered, active Utah license to practice as a Registered Nurse or Licensed Practical Nurse.
• Demonstrate clinical assessment skills at the level necessary to meet job requirements.
• Be able to make independent decisions when circumstances warrant.
• Know nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to long-term care.
• Have effective interpersonal skills and the ability to work with an interdisciplinary care team.
• Have a basic understanding of computer technology, including email use.
• Have written and oral communication skills.
• Have decision-making and analytical skills.
• Relate positively and cooperatively with employees, residents, family members, and personnel from outside agencies and organizations.
• Have a positive attitude and a desire to work with the aging population.

Responsibilities

• Assess the needs of residents in a nursing unit.
• Develop individual care plans.
• Administer nursing care to residents.
• Supervise Certified Nursing Assistants and other personnel in the delivery of nursing care.
