About The Position

Benchmark is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care to residents in a warm, comfortable, home-like environment. The role works with a team of dedicated associates and supports residents while fostering their independence. This is a full-time position with shifts from 2:00 PM to 10:00 PM or 3:00 PM to 11:00 PM. The stated wage range is $33.50–$37.50.

Requirements

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program, as required by state requirements.

Nice To Haves

• One to two years of experience working as a nurse is preferred.
• Previous paid or volunteer experience working with older adults is highly desirable.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in resident behavior and health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director, in accordance with state regulations.
• Direct and supervise the daily work assignments of Resident Care Assistants.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their jobs.
• Coordinate care needs with community providers through an effective case management process.
• Balance service demands with support for resident independence.
• Recognize and help prevent elder abuse, neglect, and exploitation, including financial exploitation, and report concerns to appropriate sources in accordance with state regulations.
• Respond to emergencies and personal emergency response system alerts, and know when to call for backup.
