Requirements

• Hold an LPN license in good standing with the Georgia Board of Nursing.
• Be certified in CPR.
• Have clinical experience; at least one year is preferred.
• Be skilled in EMR documentation and have strong organizational ability.
• Be confident working independently in a home environment.
• Demonstrate compassion, a client-focused approach, and strong communication skills.
• Have experience with G-tube, trach, or ventilator care.
• Have a valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled nursing care tailored to each client’s needs.
• Monitor health conditions and document changes in clinical status.
• Administer medications and perform wound care.
• Provide G-tube, trach, or ventilator support when required.
• Educate and guide clients and families to support ongoing health at home.
• Communicate with the care team to ensure continuity of care.
