About The Position

Concordia Visiting Nurses is seeking a dedicated and empathetic Licensed Practical Nurse to provide home health care in the Upper St. Clair area. The LPN provides skilled nursing care and support as part of the patient’s care team, helping patients with their health needs while building meaningful relationships with patients and families. The role offers autonomy in managing a schedule and prioritizing patient care, along with opportunities for training and specialization.

Concordia Visiting Nurses is a Medicare-certified, state-licensed, Medicaid-approved home health agency serving nine Western Pennsylvania counties. Its mission is to deliver high-quality care with respect, dignity, and compassion.

Requirements

• At least one year of LPN experience.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and as part of a collaborative team.
• Compassion, empathy, and emotional resilience to provide comfort and support during end-of-life situations.

Nice To Haves

• Hospice experience is preferred but not required.

Responsibilities

• Monitor patients’ vital signs.
