About The Position
Concordia Visiting Nurses is seeking a dedicated and empathetic Licensed Practical Nurse to join its home health team. The Home Health LPN provides skilled nursing care and support to patients in their homes, while helping patients and families navigate their health journey. The role offers autonomy in managing a patient schedule and prioritizing care, as well as opportunities for training and professional growth. The nurse also provides emotional support and companionship, building genuine connections with patients. Concordia Visiting Nurses is part of Concordia Lutheran Ministries, whose mission is to serve the aging community through high-quality caregiving in a Christian environment and to serve people with limited funds to the best of its ability.
Requirements
- At least one year of LPN experience.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a collaborative team.
- Compassion, empathy, and emotional resilience to provide comfort and support during end-of-life situations.
Nice To Haves
- Hospice experience is preferred but not required.
Responsibilities
- Monitor patients’ vital signs.
