About The Position

The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role includes coordinating with the RN and physician, supporting patients and caregivers with self-care, monitoring and documenting patient care, and performing physician-ordered procedures. This is a pay-per-visit role with hourly compensation for certain non-visit activities, such as orientation. The posting describes annual total cash compensation based on full-time employment at 40 weekly hours and full productivity. Compensation varies with factors including local labor markets, education, work experience, productivity, and performance. Requirements

At least 1 year of LPN or LVN experience.
Current LPN license specific to the state(s) assigned.
Current CPR certification.
Valid driver’s license, reliable transportation, and liability insurance.

Responsibilities

Verify the plan of care with the RN before each visit.
Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient’s condition changes.
Teach patients and caregivers self-care techniques.
Check vital signs.
Review medications and perform medication reconciliation at each visit.
Perform specialized procedures ordered by the physician, such as Foley catheter changes.
Prepare equipment and materials for treatments using aseptic technique.
Complete documentation accurately and on time.
