About The Position

Benchmark at Rye is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care to residents in a warm, comfortable, home-like environment. The nurse works with a team of associates and supports residents while helping foster lasting relationships within the community.

This is a full-time evening position. The posting lists the schedule as both 2p–10p in the title and 10p–6a in the body. Pay is $36–$40 per hour. Benefits eligibility may vary by employment status.

Requirements

• Current state RN or LPN license and CPR certification.
• Graduate of an approved RN/LPN program, as required by state regulations.

Nice To Haves

• One to two years of experience working as a nurse is preferred.
• Previous experience working with elderly people in a paid or volunteer position is highly desirable.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in residents’ behavior and health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director, in accordance with state regulations.
• Direct and supervise the daily work assignments of Resident Care Assistants.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their work.
• Coordinate care needs with community providers through an effective case management process.
• Balance service demands with support for resident independence.
• Recognize and help prevent elder abuse, neglect, and exploitation, and report concerns to appropriate sources as required by state regulations.
• Respond to emergencies and personal emergency response system alerts, and determine when backup is needed.
