The Licensed Practical Nurse works under the direct supervision and responsibility of a clinic provider in a TGHN clinic or the hospital nursing leader on a nursing unit. Using a team-based approach, the LPN assists staff and providers with patient care, clerical, and organizational tasks and provides patients with information to help them use and benefit from care services. The LPN may be assigned to a medical specialty or may cross-cover multiple specialties based on competency. The role works closely with a supervisor to complete responsibilities in a timely manner. The LPN contributes to patient care through observation, nursing diagnosis, planning, intervention, and evaluation; health teaching and counseling; and the promotion of wellness, health maintenance, and illness prevention. The role administers medications and treatments as prescribed or authorized by a practitioner licensed to do so under state law, and may supervise and teach other personnel in relevant nursing activities. The LPN is responsible for providing competent care, clarifying assignments with management, assessing personal skills and knowledge, and exercising informed judgment and decision-making within the LPN scope of practice. The professional nurse collaborates with staff, physicians, and patients on programs to reduce Hospital Acquired Conditions and Hospital Acquired Infections. The LPN uses SBAR for handoff communication within and between departments and includes patients and families in bedside shift reports. The LPN escalates concerns as needed to support safe, timely patient care and prepares equipment and assists physicians during treatments and examinations. The role may require working beyond the regular tour of duty or in another patient care unit as assigned by the department head or nursing supervisor.
