About The Position

The Licensed Practical Nurse (LPN) provides practical nursing services under the supervision of a Registered Nurse. The position accepts client assignments from the R.N. Case Manager and provides care using home assessments, nursing diagnoses, and physician-approved care plans. The LPN performs physician-ordered treatments delegated by the R.N. Case Manager, monitors and reports client signs, symptoms, and changes in physical or emotional status, and promotes independence in self-care for clients and caregivers. The role uses electronic documentation and follows Agency documentation, universal precautions, and safety policies. The LPN also completes other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.

Requirements

Hold a current Licensed Practical Nurse license in Virginia.
Have a minimum of one year of nursing experience.
Have current CPR certification.
Have excellent communication, organizational, and human relations skills.
Be able to function with minimal supervision.
Be able to manage multiple projects and changing priorities.
Be able to employ critical thinking skills.

Responsibilities

Accept assignments of clients from the R.N. Case Manager.
Provide nursing care using home assessments, nursing diagnoses, and physician-approved care plans.
Perform treatments ordered by the physician and delegated by the R.N. Case Manager.
Observe signs and symptoms and report them to the R.N. Case Manager.
Note changes in clients’ physical or emotional status.
Promote client and caregiver independence in self-care.
Use electronic documentation of client care and follow Agency policies for timely submission and data synchronization.
Follow Agency policies regarding universal precautions and safety guidelines.
Fulfill other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.
