About The Position

The Licensed Practical Nurse (LPN) will administer medications safely and accurately, assist the RN with patient assessments and reassessments, and implement appropriate rehabilitation nursing interventions. The role involves performing timely Quality Improvement (QI) assessments under RN supervision, delivering care based on patient response, and updating the interdisciplinary plan. The LPN will also assess and reassess pain, implement pain management techniques, and educate patients and families. Developing and implementing individualized teaching plans based on patient and family learning needs is also a key responsibility. The position requires strong clinical skills, a commitment to safe, patient-centered care, and the ability to work effectively in an interdisciplinary team environment.

Requirements

  • Graduate of an accredited Licensed Practical/Vocational Nurse program.
  • Current state licensure as a Licensed Practical/Vocational Nurse.
  • CPR/BLS certification required.
  • IV Certification highly preferred or must be obtained within 6 months.

Nice To Haves

  • Minimum one year of Medical/Surgical experience in an acute care setting preferred.
  • IV Certification

Responsibilities

  • Administer medications safely, accurately, and based on patient age and clinical condition.
  • Minimize potential for medication errors by following proper medication administration procedures.
  • Assist the RN with initial assessments and patient reassessments per policy.
  • Identify and implement appropriate rehabilitation nursing interventions.
  • Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
  • Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
  • Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
  • Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
  • Develop and implement individualized teaching plans based on patient and family learning needs.
  • Perform other duties as assigned.
